JOB SUMMARY: Works directly with patients and other staff, including nurses and physicians. Calls or sees all eligible Medicare patients to enroll into the Case Management Program. Various office duties performed as needed. EDUCATION: High School Diploma required. Graduate of an accredited program for Licensed Practical Nurses preferred. QUALIFICATIONS: Two years office experience. Case management experience preferred. Must possess skill in human relations, professional diplomacy, tact and sensitivity required to interact with persons involved.
