POSITION AVAILABLE: Director of Social Services – LMSW OR LCSW

JOB SUMMARY: Coordinates clinical patient services and provides efficient resource utilization to assure optimal clinical quality outcomes, including family, psychosocial, and discharge planning needs. Provide persons, families, or vulnerable populations with the psychosocial support needed to cope with chronic, acute, or terminal illnesses. Serves as a liaison with community resources. Provides services include advising family care givers, providing patient education and counseling, and making necessary referrals for other social services in a courteous and customer-focused manner.

QUALIFICATIONS: Master Social Work Degree
